Clinical Documentation 101

I HATE writing notes.. and you know you do too! We went to school to be clinicians, NOT authors!! And while in school, we learned all about clinical theories, diagnosis, interventions, etc., but clinical documentation was not part of any of our programs. Now here we are, most of us in private practice, writing notes the way we think they should be written with very little guidance from insurance companies.

If you are in private practice and you accept insurance then you NEED to know how to write notes. Insurance companies can audit your charts at anytime and if your documentation doesn't meet their guidelines then you'll be giving back your hard earned money.. and A LOT of it!!

Learning Objectives

Learn to write progress notes, treatment plans, assessments, & discharge plans that are in compliance with insurance criteria

Decrease the amount of time spent writing notes

Identify red flags that might trigger an insurance audit

Learn what steps to take if you are audited

Learn all about..

Assessments

  • Assessing, diagnosing, and documenting to justify medical necessity
  • How often to assess
  • How to write a diagnostic summary to justify treatment
  • Connecting the diagnosis to treatment planning

Treatment Plans

  • Everything that is needed in a treatment plan and treatment plan updates
  • Frequency of writing treatment plans
  • Assessing and documenting progress
  • Understanding the difference between goals, objectives, and interventions

Progress Notes

  • Everything that's needed in a session note
  • Documenting for medical necessity
  • How to justify an increase in session frequency
  • How to maintain client confidentiality

Discharge Notes

  • Why they're important and when to do them
  • Everything that is needed in a discharge note
  • Sending termination letters
  • Legal and ethical considerations

This training is for psychotherapists who want to improve their documentation skills and knowledge.

Whether you're licensed, provisionally licensed, an intern, working in private practice or an agency, you need to know how to write notes.

NASW approved for 3 CEUS for LCSW, LPC, LMFT, and PsyD

for CT licensed clinicians.

**CEUs will be offered for additional states in 2023**

What are they saying???

"Hi Shalyn, I just wanted to take a minute and thank you for offering such a great training! I really appreciate all the information, handouts, personal stories, etc. that you shared. I found it so helpful and would highly recommend it to anyone in the future. I’m still processing some of what you shared and keep thinking to myself how lucky your clinicians

are to be able to learn this directly from you."

-Jess T.

"Thank you for a great training. I feel so much more confident about writing notes and have already started implementing changes. You're a really good teacher. Thanks again!"
